Adjuvant nivolumab for resected gastroesophageal cancer

Neoadjuvant chemoradiotherapy followed by resection is widely used for patients with locally advanced oesophageal or gastroesophageal junction cancer, but risk of recurrence is high. A global, randomized, double-blind phase III trial assessed the use of adjuvant nivolumab, a fully human monoclonal anti- PD1 antibody, in patients who received neoadjuvant chemoradiotherapy followed by oesophageal or gastroesophageal junction cancer resection and who were at high risk for recurrence. 532 patients received nivolumab (240 mg every two weeks, then 480 mg every 4 weeks, for up to 1 year) and 262 received placebo. Median follow-up was 24.4 months. Median disease-free survival was 22.4 months (95% CI 16.6–34.0) and 11.0 months (95% CI 8.3–14.3) in the nivolumab group and the placebo group, respectively. Hazard ratio for disease recurrence or death was 0.69 (96.4% CI 0.56–0.86; P < 0.001). Grade 3 or 4 adverse events related to treatment occurred in 13% (nivolumab) and 6% (placebo) of patients.
